Sole Source Justification Questionnaire

NSSD Purchasing Policy H-27 and Utah Code §63G-6a-802 require a documented determination before approving a sole-source purchase. A preferred brand or previous purchase from the same vendor does not, by itself, establish sole source.

Explain what you need, why only one supplier can meet that need, and what alternatives you have considered. Upload your requisition, vendor quote, and any supporting documents.
