  • Immunization Record: Immunization information can be found under Health Services on our website.
  • Proof of Residency: Documentation showing your physical address within our district boundaries (i.e., mortgage statement, rental agreement, utility bill, etc.).
  • Legal Documents: Any legal document outlining custody or parenting plans, if applicable.
  • Current copy of IEP or 504, if applicable.

RETURNING STUDENT REGISTRATION

Each school year it is necessary for parents to update their student registration forms. It

is CRITICAL for school administration to have the most current data for your students in

order to keep your students safe and to keep parents informed. In addition, students

must have parent authorization for our Student Internet Acceptable Use Policy to

access school computers each year. This authorization is completed during the online

registration process.

Please update your student’s forms no later than August 9, 2024. You may access our

SchoolForms registration system from home through your parent PowerSchool account.

An Online Registration Help Document (English | Spanish ) is available to guide you. If you do not have internet access, you may still register in person. Someone will be available to help you through the process during summer

registration dates to be posted soon.

KINDERGARTEN AND PRESCHOOL REGISTRATION

If you have a child turning five years of age on or before September 1 of the upcoming

school year, he or she qualifies for next year’s kindergarten program. If your child

qualifies, please contact your local school at the email listed below to make sure they

are on the enrollment list.

North Sanpete School District offers preschool for students age three and four. Priority

will be given to four-year old students (age 4 on or before September 1 of the upcoming

school year). Three-year old students (age 3 on or before September 1 of the upcoming

school year) will be considered if space is available. Classes fill up extremely quick.

Please email your school to get your child on the enrollment list.

ALL KINDERGARTEN CLASSES will  be attending full-day in ALL schools! Full-day kindergarten not only boosts students academic achievement, it also strengthens their social and emotional skills. Parents

who choose a half-day option can do so, however classes will be in the morning and no

transportation in the middle of the day will be provided.

Please review the required forms for new student registration above PRIOR to your

student attending school.

North Sanpete School District does not discriminate on the basis of race, color, national

origin, sex, disability, or age in its programs.
