NOTIFICATION OF RIGHTS UNDER THE PROTECTION OF PUPIL RIGHTS AMENDMENT (PPRA)/UTAH FERPA (UFERPA)

PPRA / Surveys / Optional Programs

As part of educational programs, student wellness efforts, school improvement planning, and state or federal reporting requirements, students may occasionally be asked to participate in surveys, evaluations, screenings, or other information collection activities. North Sanpete School District recognizes that some of these activities may involve topics families consider personal or sensitive.

Federal law provides parents with important rights regarding these activities, including the right to receive notice, review materials, and opt students out in certain situations. This notice is provided in accordance with the Protection of Pupil Rights Amendment (PPRA).

Parent Rights Under PPRA

Under federal law, parents and guardians have the right to receive notice and, when applicable, inspect certain instructional materials, surveys, and information collection instruments used with students. Parents also have the right to opt their student out of participation in certain activities described below.

Surveys Involving Protected Information

The District will provide notice and, when required by law, an opportunity to opt out before students participate in surveys funded in whole or in part by the U.S. Department of Education that ask about one or more of the following protected areas:

  • Political affiliations or beliefs
  • Mental or psychological problems of the student or the student's family
  • Sexual behavior or attitudes
  • Illegal, anti-social, self-incriminating, or demeaning behavior
  • Critical evaluations of family members
  • Legally recognized privileged relationships, such as with attorneys, physicians, or clergy
  • Religious beliefs, practices, or affiliations
  • Income information, other than information required by law to determine eligibility for programs or financial assistance

Instructional Materials

Parents have the right to inspect instructional materials used as part of the educational curriculum. Instructional materials may include:

  • Printed materials
  • Digital content
  • Videos or presentations
  • Supplemental curriculum resources

Requests to review instructional materials should be directed to the student's school.

Administration of Surveys

The District may administer surveys or questionnaires for purposes such as:

  • School climate and culture
  • Student wellness and support services
  • Program evaluation and improvement
  • College and career readiness
  • State or federal reporting requirements

Whenever possible, survey data will be collected and reported in aggregate form rather than identifying individual students. Participation requirements and opt-out opportunities, when applicable, will be communicated before administration.

Annual and Activity-Specific Notice

This notice serves as the District's annual PPRA notification. Additional notice will be provided when specific activities or surveys require parent review opportunities or opt-out rights under federal law.
